What an aftercare plan can include

No two plans look alike. Yours is shaped by your history, mental health needs, home environment, work and support system.

  • Continued clinical care

    Arrangements with identified providers for ongoing therapy, psychiatric and medical follow-up and, when appropriate, a step-down level of care.

  • Recovery community

    A 12-step meeting schedule, a sponsor or peer groups that offer honest encouragement and accountability.

  • Home, work and relationships

    A plan for boundaries, family conversations, returning to work and, if needed, a sober living environment.

  • Relapse prevention

    Your personal warning signs and the specific steps you will take when a hard day comes.

  • Daily wellness

    Small, repeatable habits around sleep, meals, movement and stress that support stability.

  • Practical resources

    Community resources and, if you need more support, recommendations for extended care or sober living housing.

Your relapse prevention plan

Relapse prevention is not a test of willpower. It is a written plan you can follow when stress makes it hard to think clearly.

  1. Know your warning signs

    Loneliness, anger, grief, overconfidence, certain places or people. You learn to recognize your own triggers during treatment.

  2. Write down your steps

    Leave the situation, call a sponsor or therapist, attend a meeting or reach out to a supportive family member.

  3. Keep your people close

    Your plan lists exactly who to contact, so help is one call away.

  4. If a lapse happens, reach out quickly

    A lapse does not erase recovery. Safety and reconnecting with support come first, without shame.

Support for alumni

Support when you need it after discharge

After you leave, Harlow Gardens alumni can return for aftercare groups and individual sessions, including support in a crisis or after a lapse. These services are offered for a fee. The goal is to help you rely more and more on your own recovery community.

When does aftercare planning start?

At admission. Your clinical team begins planning for continuing care as part of treatment, so you have time to practice the plan before you leave.

What if I live outside the Coachella Valley?

Your plan is built around where you will live. Your team helps identify therapists, outpatient programs, meetings and support groups close to home.

Do I need to go to outpatient treatment after residential care?

It depends on your needs. Some people benefit from a step-down level of care, while others are ready for outpatient therapy and a strong recovery community. Your team will recommend what fits your situation.

What if I relapse after leaving?

Reach out for help right away. A lapse does not erase your progress, and getting support quickly matters. Alumni can return for individual sessions or aftercare groups, which are offered for a fee. In an emergency, call 911, or call or text 988 for crisis support.

Can my family be part of the plan?

When it is appropriate and you agree, yes. Families can take part in family therapy and help prepare for life at home. Learn more for families.
